If you are doing in person lessons, you get a book at the start of your first lesson and it is filled up over time with your unique lesson plans. What you learn in each lesson gets put into your book so that you know what to practice.
Online students can purchase the book written by the founder of Guitar Education. It is the best book to get started with for anyone who is a beginner.

That depends on you. The amount of time you practice everyday will determine how good you get and how fast you'll get there. More practice = faster results.
